Parent Forms

For the safety of our students, we ask all families to let us know who will be responsible for pick-up and drop-off. Please list all authorized individuals below. Only the names provided on this form will be permitted, unless we receive written notice of changes. Our staff may request photo ID at pick-up to verify authorization. Thank you for helping us keep your child safe and cared for during transitions.
